Know your insurance benefits! Some plans do not cover immunizations, hearing tests, vision tests and lab work. Please go on-line with your insurance to verify your coverage....BEFORE your appointment!

Please have your card with you at the time of your visit. If we file your claim, you are responsible for only the co-pay at the time of visit. If we are not a provider for your insurance, if you are not insured or we do not file with your insurance company, you may be responsible for the entire charge at the time of service. Check your insurance website for a provider list.

We accept cash, checks, VISA, Discover and MasterCard.

Hospitals

If your child needs hospitalization, we have privileges at:

Avista Hospital, Louisville, CO
Boulder Community Foothills Hospital, Boulder, CO
Exempla Good Samaritan Medical Center, Lafayette, CO

We can admit your child for specialist care to other facilities if necessary. We use either The Children’s Hospital or Rocky Mountain Hospital for Children. After the admission, please follow your insurance company requirements to reduce your risk of payment problems.
